SALAM Islamic Center Liability Waiver for Sisters Only Fitness Class
Acknowledgment of Risks:
Participation in physical activities, such as the Sisters Only Fitness Class, inherently involves the risk of physical injury. This could include, but is not limited to, muscle strains, ligament tears, broken bones, and fatigue. While SALAM Islamic Center takes every reasonable precaution to ensure the safety of all participants, it is impossible to eliminate all risks associated with physical activity.
Assumption of Risk:
By signing this waiver, I, the undersigned participant, acknowledge and accept the risks associated with the fitness class. I accept personal responsibility for any bodily injury, accident, illness, loss of personal property, or other consequences resulting from my participation in the class.
Health and Safety:
I affirm that I am eighteen (18) years of age or older and in good health. I am participating in the Sisters Only Fitness Class with the knowledge that doing so is medically safe for me. I agree to disclose any physical limitations, disabilities, ailments, or impairments that may affect my ability to participate in the class.
Compliance with Instructions:
I agree to comply with all instructions, rules, and guidelines presented by SALAM Islamic Center and the fitness instructor, Sr. Farida. I acknowledge that failure to follow these guidelines may increase the risk of injury and can also lead to my removal from the class.
Release of Liability:
I hereby release, waive, discharge, and covenant not to sue SALAM Islamic Center, its directors, officers, employees, volunteers, and agents from any and all liabilities, claims, demands, causes of action, and expenses arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in such activity.
Understanding of Terms:
I have read this release of liability and assumption of risk agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
